This form allows staff of member organizations to register themselves for a CIP4 website and email account and password (e.g., "account") and allows members with active accounts to register members from their company or from other member organizations for an account. In all cases completed forms are first sent to the primary contact for the appropriate member organization for approval. Nonmembers cannot use this form to register for an account, and they should complete the member application.
